Who Do We Serve?                

The program is designed for seniors who have a chronic health problem or who have difficulty meeting their own social needs.

Standard

All clients admitted to the centre must meet the admission criteria.

Suitable Clients

Individuals

bulletwho are socially isolated;
bulletwho without this service, would deteriorate in physical, mental, social and/or emotional functioning and would likely receive facility placement;
bulletwho because of physical, mental, and/or emotional functioning, are not suitable for attendance at other community programs.
bulletwho require restorative or rehabilitative services in order to achieve an optimal level of functioning.
bulletwhose care needs can be met in a group setting;
bulletwhose behaviour does not diminish other's enjoyment of the program day;
bulletwho possess the necessary stamina to manage a typical Adult Day Program day;
bulletwho are capable of participation in Adult Day Program activities.
bulletwho appear to be capable of attending other community programs but, due to extenuating circumstances such as the following, require Adult Day Centre services:
bulletSevere isolation: defined as those "who have withdrawn from community life and whose mental and/or physical health is deteriorating from lack of social relationships and meaningful activities";
bulletUnique physical or mental needs: e.g. the client who displays challenging behaviour that requires a special approach to function in a group setting.

 

Referrals

Referrals can come from any source, but eligibility must be determined by a Long Term Care Assessor.

Membership Boundaries

ASK primarily serves the Vancouver neighbourhoods of Arbutus, Kerrisdale, Dunbar, and Shaughnessy.

For those who want to join, our general boundaries are:
bulletSouth to the Fraser River,
bulletNorth to 16th Avenue,
bulletEast to Oak and Granville Streets
bulletWest to Camosun Street (near Dunbar Street)

These boundaries exist because there are other services in the neighbouring areas and because our driving routes to bring members in need to be manageable. (Nobody wants to sit any longer than necessary on the bus while we go about picking up our members.)
