| | Who Do We Serve?
The program is designed for seniors who have a chronic health problem or who
have difficulty meeting their own social needs.
All clients admitted to the centre must meet the admission criteria.
Suitable Clients
Individuals
 | who are socially isolated; |
 | who without this service, would deteriorate in physical, mental, social
and/or emotional functioning and would likely receive facility placement; |
 | who because of physical, mental, and/or emotional functioning, are not
suitable for attendance at other community programs. |
 | who require restorative or rehabilitative services in order to achieve an
optimal level of functioning. |
 | whose care needs can be met in a group setting; |
 | whose behaviour does not diminish other's enjoyment of the program day; |
 | who possess the necessary stamina to manage a typical Adult Day Program
day; |
 | who are capable of participation in Adult Day Program activities. |
 | who appear to be capable of attending other community programs but, due to
extenuating circumstances such as the following, require Adult Day Centre
services:
 | Severe isolation: defined as those "who have withdrawn from
community life and whose mental and/or physical health is deteriorating
from lack of social relationships and meaningful activities"; |
 | Unique physical or mental needs: e.g. the client who displays
challenging behaviour that requires a special approach to function in a
group setting. |

Referrals can come from any source, but eligibility must be determined by a
Long Term Care Assessor.
These boundaries exist because there are other services in the neighbouring areas and because our driving routes to bring members in need to be manageable. (Nobody wants to sit any longer than necessary on the bus while we go about picking up our members.) |
